Feedback Widget stays at the top of the page ... If users are at the bottom of the page, in the middle or anywhere but the top of the page and click on the "Leave Feedback" button they will not see the widget or even realize it has already come...

When widget pops up it doesn't stay centered when scrolling up/down. While the widget is visible it would be a good idea to keep it centered till the widget is closed.

You should add a black transparent background behind the widget so it covers the site. This way you get the users to focus more on the widget.
